Adalimumab
Indication
Monitoring of the Adalimumab level and control of Anti-Drug Antibodies (ADA) against the drug.
Method
Note! From May 1, 2024, concentration determination is performed using a new method (ELISA), which is comparable to the previous method. From May 1, 2024, determination of anti-drug antibodies is performed with a new method (RIA). Unlike the previous method, the new method is quantitative.
Concentration determination is performed with ELISA, in serum. ADA analysis is performed with RIA, in serum.
Result
Drug concentration is reported as a concentration (µg/mL). Measurement range: 0.01 - 250 µg/mL
ADA is reported as a concentration (AU/mL). ADA <12 AU/mL are not detectable, 12-30 AU/mL are detectable but not quantifiable and >30 AU/mL can be quantified.
Interpretation
The sample should be drawn at trough value. Low or unmeasurable concentration of the biologic pharmaceutical is associated with poor clinical effect. One explanation can be that antibodies against the drug (anti-drug antibodies, ADA) block the effect and enhance clearance of the drug. At drug concentrations <5 μg/mL it is thus recommended to further test for ADA. Presence of drug in the test sample may prevent the detection of ADA.

Monitorering av Adalimumab-koncentration samt kontroll av eventuella anti-läkemedelsantikroppar (ADA) mot läkemedlet.
Metod
Obs! Fr.o.m. 1 maj 2024 utförs koncentrationsbestämning med en ny metod (ELISA) som är jämförbar med den tidigare metoden. Fr.o.m. 1 maj 2024 utförs bestämning av anti-läkemedelsantikroppar med en ny metod (RIA). Till skillnad från tidigare metod är den nya metoden kvantitativ.
Koncentrationsbestämning utförs med ELISA i serum. ADA-analys utförs med RIA i serum.
Resultat
Läkemedelskoncentrationen anges som koncentration (µg/mL). Mätområde: 0.01 - 250 µg/mL
ADA anges som koncentration (AU/mL). ADA <12 AU/mL kan inte detekteras, 12-30 AU/mL kan detekteras men inte kvantifieras och >30 AU/mL kan kvantifieras.
Tolkning
Provet bör tas vid dalvärde. Låg eller omätbar koncentration av biologiskt läkemedel är associerat med sämre klinisk effekt. En viktig orsak kan vara att antikroppar mot läkemedlet (anti-läkemedelsantikroppar, ADA) har bildats och blockerar dess effekt och påskyndar clearance av läkemedlet. Vid läkemedelskoncentration <5 μg/mL rekommenderas därför komplettering med analys av ADA. Om läkemedlet kan påvisas i provet kan ADA inte säkert detekteras.
